What goes wrong

The Twin Cam 88's spring-loaded cam chain tensioner shoes wear through and dump debris into the oil system, and the outer cam bearings are a documented failure point on the same engines. Together these are the defining risk of the era, and by now every one of these engines is old enough to have gotten there.
